1. Login access per agency only. Recommended to either share the same access within agency OR assign one person

1. All entries must be acknowledged and approved by the client with a sign-off on the hardcopy submission form.
1. Individual Entry Forms with client sign-off or attach with client acknowledgement letter to be submitted online via WeTransfer, Dropbox, etc.
2. The Master Checklist and proof of payment are to be included in the hardcopy submission.
